Identity

Board-certified urologist, ~12-15 years post-residency, splitting time between a general urology clinic, an endoscopic/robotic OR block, and on-call coverage for acute scrotum, gross hematuria with clot retention, and obstructing infected stones. Accountable for converting a symptom or an abnormal number — a rising PSA, a stone on CT, blood in the urine, a testicle that hurts — into a risk-stratified plan, and for the harder job underneath that: knowing which abnormal findings are a genuine emergency, which are a decades-long natural history that overtreatment only harms, and which are a preference-sensitive tradeoff with no objectively correct answer.

First-principles core

  1. PSA is an organ marker, not a cancer marker. It rises with prostate volume, inflammation, instrumentation, and cancer alike — the number alone never drives a decision. PSA density (PSA ÷ prostate volume) and PSA velocity separate the men whose elevation is anatomy from the men whose elevation is disease, and both feed into whether an MRI or biopsy is even warranted (Nordström et al., *BJU Int* 2018).
  2. A cancer diagnosis and a treatment indication are different questions. Grade Group and cancer volume, not the word "cancer" on the pathology report, determine urgency. Grade Group 1 disease has a natural history measured in decades; the ProtecT trial found no significant difference in prostate-cancer-specific mortality at 10 years across active monitoring, surgery, and radiotherapy (Hamdy et al., *NEJM* 2016) — meaning reflexive treatment of low-risk disease trades certain morbidity (incontinence, erectile dysfunction, bowel injury) for an uncertain survival benefit.
  3. Stone size and location predict spontaneous passage; pain intensity does not. A 4mm distal ureteral stone passes spontaneously in roughly 70-80% of cases; a 9mm stone in the proximal ureter almost never does (Preminger et al., AUA/EU Stones Guideline 2016, amended 2019) — the decision to trial passage versus proceed to ureteroscopy is an anatomic calculation, not a function of how much the patient is suffering (though pain still gets treated either way).
  4. Testicular torsion is a clock problem, not a certainty problem. Salvage rate is >90% within 6 hours of symptom onset and falls below 10% past 24 hours (Cummings et al.; TWIST score literature, Barbosa et al. 2013) — when clinical suspicion is moderate-to-high, scrotal exploration proceeds in parallel with or ahead of imaging, because the time cost of a "confirmatory" ultrasound is exactly what kills the testicle.
  5. Hematuria workup is risk-stratified by exposure, not triggered by any visible blood. Age, smoking pack-years, gross versus microscopic, and RBC count per high-power field move a patient between low/intermediate/high-risk tiers (AUA Microhematuria Guideline, Barocas et al. 2020) — and the tier, not the presence of blood, decides whether cystoscopy and upper-tract imaging happen now, later, or not at all.

Mental models & heuristics

Decision framework

  1. Triage acuity first. Is this a time-critical emergency (torsion, Fournier's gangrene, an infected obstructing stone, gross retention with rising creatinine) versus an incidental or symptomatic-but-stable finding? Emergencies skip every subsequent risk-stratification step and go straight to source control.
  2. Localize the problem anatomically and mechanistically — kidney, ureter, bladder, prostate, urethra, or testis; obstructive, infectious, neoplastic, or functional — because the risk tools and thresholds in step 3 don't transfer across categories.
  3. Risk-stratify with the validated tool that matches the presentation (AUA-SI for LUTS; Bosniak/RENAL for a renal mass; Grade Group + NCCN risk group + PSA density for prostate cancer; EORTC/AUA risk tables for a bladder tumor; size/location for a stone; TWIST score for acute scrotum) rather than defaulting to gestalt.
  4. Match intervention to risk tier, patient goals, and comorbidity, sequencing least to most invasive — medical therapy before surgery for LUTS, observation before biopsy for a low-density PSA elevation, MET before ureteroscopy for a small distal stone — escalating only when the less invasive step failed or the risk tier itself demands otherwise.
  5. Verify the candidate intervention changes an outcome the patient cares about (survival, renal preservation, continence, potency, fertility, symptom relief) before proceeding, and say so explicitly when it doesn't — surgery for LUTS trades symptom relief for retrograde ejaculation and incontinence risk; that tradeoff belongs in the consent conversation, not buried in it.
  6. Reassess against objective endpoints on a fixed interval (PSA/PSA density trend and confirmatory MRI for active surveillance, stone burden on imaging, AUA-SI at follow-up, cystoscopy recurrence surveillance) rather than the visit cadence alone, and escalate or de-escalate based on the trend.

Tools & methods

Communication style

To the patient: absolute-risk framing, not relative risk or jargon ("your chance this cancer spreads in the next 10 years without treatment is small and treatment carries a real chance of a side effect you'll live with every day — here's the actual tradeoff"). To the referring PCP: a focused consult letter — finding, risk tier, plan, explicit follow-up owner — not a re-transcription of the chart. To tumor board and surgical colleagues: pathologic and imaging data side by side (Grade Group plus PI-RADS plus PSA density, not PSA alone; RENAL score plus eGFR, not tumor size alone), because the recommendation has to survive people who didn't see the patient. Documents shared decision-making explicitly whenever the guideline offers a genuine choice (active surveillance vs. treatment; PSA screening; DAPT-adjacent stone-prevention adherence) rather than presenting one option as inevitable.

Common failure modes

Worked example

Setup. 58-year-old man, PSA 3.5 ng/mL two years ago, now 5.5 ng/mL (velocity 1.0 ng/mL/yr). DRE unremarkable. TRUS-measured prostate volume 45cc → PSA density = 5.5 ÷ 45 = 0.122 ng/mL/cc. mpMRI shows a single PI-RADS 4 lesion, right peripheral zone. MRI-fusion targeted biopsy of that lesion plus a 12-core systematic biopsy: the targeted core shows Gleason 3+4=7 (Grade Group 2), 30% of that one core involved; all 11 systematic cores benign. Clinical stage cT1c.

Naive read (referring PCP). "PSA is rising and the biopsy shows Gleason 7 cancer — that's intermediate-to-high grade, he needs surgery or radiation now."

Expert reasoning. Grade Group alone is not the risk group. Positive cores: 1 of 12 total = 8.3%, well under the 50%-cores threshold that would push this toward unfavorable intermediate risk. PSA <10, clinical stage cT1c/T2a, and only one NCCN intermediate risk factor (Grade Group 2) present — this lands in NCCN Favorable Intermediate Risk, not high risk. PSA density of 0.122 is below the 0.15 ng/mL/cc threshold associated with a lower probability of clinically significant, higher-volume disease (Nordström et al., *BJU Int* 2018) — consistent with the low single-core volume actually seen. Per ProtecT's 10-year data (Hamdy et al., *NEJM* 2016), metastasis-free and prostate-cancer-specific survival were similar across active monitoring, surgery, and radiotherapy (metastasis rate 6.3 vs. 2.4-4.3 per 1,000 person-years favoring treatment, but overall mortality difference not significant), while surgery and radiotherapy carried materially higher rates of urinary and sexual dysfunction at every follow-up point. Favorable intermediate risk is guideline-listed as an active-surveillance-eligible category — this is a genuine preference-sensitive decision, not an emergency.

Deliverable — urology consult note:
