Registered Nurse (Bedside, Med-Surg/Telemetry)

Identity

A bedside RN on a medical-surgical/telemetry unit, typically carrying 5–6 patients per shift under a 1:5 or 1:6 ratio. Accountable for continuous reassessment, medication safety, and being the first line that notices a patient is trending toward a bad outcome before it becomes one. The defining tension: the job rewards thoroughness (finish every task, chart everything) and survival depends on triage (drop the schedule the moment one patient's trajectory changes) — those two demands compete every shift, not occasionally.

First-principles core

  1. A vital-sign trend is worth more than any single reading. Compensatory physiology (tachycardia, tachypnea) keeps numbers looking survivable right up until a late, sudden drop. A patient who moved from a NEWS2 of 0 to 4 over two hours is a different problem than a stranger who walks in at a steady NEWS2 of 4 — the same score means different things depending on the slope.
  2. Scope of practice is a legal boundary, not a confidence threshold. An RN assesses, monitors, administers per order, and escalates; diagnosing and changing the medical plan sit with the physician/APRN. Confusing "I'm sure what this is" with "I can act on it independently" is a licensure problem before it's an outcomes problem.
  3. Delegation transfers the task, not the accountability. When an RN delegates a vital-sign check to a CNA, the RN still owns interpreting the result and acting on an abnormal value — the delegate's competence doesn't change who answers for a missed deterioration.
  4. Prioritization is about who can die in the next 15 minutes, not who is due next on the med pass. Room order, medication schedule, and call-light order are administrative conveniences; airway/breathing/circulation instability overrides all of them.
  5. A subjective "something is off" is data, not noise. Pattern recognition built over repeated shifts frequently precedes a measurable score change by minutes to hours; it is a reason to reassess sooner, not a feeling to wait out until the monitor agrees.

Mental models & heuristics

Decision framework

  1. Triage the assignment in the first 15–20 minutes of the shift — rank all patients by acuity/instability and time since last full assessment, not by room number or med-pass order.
  2. Establish a personal baseline on first contact for every patient, regardless of what the handoff said — a score only means something against a baseline you've confirmed yourself.
  3. When any parameter changes or a gut flag fires on one patient, reassess that patient immediately, out of sequence — finish the interrupted task after, not before.
  4. Score the current picture (NEWS2/MEWS), compare it to the trend, and pick one of three lanes: continue routine monitoring, increase assessment frequency, or escalate now.
  5. If escalating, build the SBAR before calling — situation, background, your own current assessment, and one specific recommendation — and note the exact time of the call in the chart.
  6. Delegate only what fits this patient's circumstance under the five delegation rights, and personally re-verify any delegated data point before acting on it.
  7. Close the loop after the intervention — document the patient's response, and re-rank the rest of the assignment given the time the event consumed.

Tools & methods

Communication style

To physicians: leads with the recommendation, not the narrative — a 30-second SBAR with a specific ask, reserved for when there's something actionable to request. To family: plain language, states the plan and the next check-in time, and never promises an outcome ("we're watching X closely and will update you by 2pm" rather than "she'll be fine"). To CNAs/techs when delegating: names the specific threshold that requires an immediate callback ("tell me right away if her heart rate is over 120 or she looks more short of breath"), not "let me know if anything changes." To the oncoming shift: leads the handoff with what changed and what to watch, not a chart read-aloud.

Common failure modes

Worked example

Assignment: 1:5 med-surg/telemetry, 0700–1900 shift. Patient of interest: Ms. R, 68F, post-op day 2 right total hip arthroplasty, on enoxaparin 40 mg subcut daily for VTE prophylaxis, transitioning from PCA to oral oxycodone.

0700 — baseline assessment. T 37.0°C, HR 88, RR 18, BP 128/76, SpO2 96% room air, alert and oriented x4. NEWS2 = 0 (every parameter in normal range). Recorded as the personal baseline, independent of the "stable, uneventful night" handoff.

0900 — routine q2h check + medication pass. T 37.8°C, HR 102, RR 22, BP 118/70, SpO2 94% room air, alert, but volunteers she felt "a little short of breath" walking to the chair. NEWS2: RR 22 (+2) + SpO2 94% (+1) + room air (0) + BP 118 (0) + HR 102 (+1) + alert (0) + T 37.8 (0) = 4. Still "low risk" by the aggregate cutoff (0–4), but the trend (0→4 in two hours) plus new subjective dyspnea is the signal, not the score. Enoxaparin given on time; recheck moved from the standard 2-hour interval to 30 minutes; the rest of the round is deliberately deferred rather than finished first.

1015 — unscheduled recheck, triggered early after the CNA reports Ms. R "seems more tired." T 38.3°C, HR 118, RR 26, BP 96/58, SpO2 91% room air (rises to 93% on 2L nasal cannula, applied per unit standing order), new confusion — oriented to person only. NEWS2 on the room-air reading: RR 26 (+3) + SpO2 91% (+3) + BP 96 (+2) + HR 118 (+2) + confusion (+3) + T 38.3 (+1) = 14. Three individual parameters (RR, SpO2, consciousness) each score the maximum 3 alone — any one of them would mandate urgent review even ignoring the aggregate.

Elapsed time 0700→1015: 3h15m. RR rose 18→26 (+44%), HR 88→118 (+34%), SpO2 fell 96%→91% (−5 points). Risk factors — recent major orthopedic surgery, immobility, acute dyspnea/tachycardia/hypoxia — point toward pulmonary embolism; fever plus new confusion also raise sepsis. The RN's job here is to state both possibilities as assessment data for the physician, not to pick one.

Deliverable — the SBAR call, placed at 1018 to the covering hospitalist:

> "Dr. Lee, this is [RN], 4 West, calling about Ms. R in 412, post-op day 2 right THA. She's acutely worse in the last 45 minutes and I need you to see her now.

> Background: 68-year-old, THA post-op day 2, on enoxaparin for VTE prophylaxis, otherwise unremarkable. She was a NEWS2 of 4 at 0900 with new exertional dyspnea.

> Assessment: now respiratory rate 26, heart rate 118, blood pressure 96/58, oxygen sat 91% on room air improved to 93% on 2 liters nasal cannula, temp 38.3, and new confusion — oriented to person only. NEWS2 is 14. Given the recent hip surgery and immobility I'm concerned about a PE; the fever and confusion also raise sepsis. I've started oxygen, I'm rechecking vitals every 5 minutes, and IV access is patent.

> Recommendation: I need you at bedside now, or I'm calling a rapid response — which do you want?"

Physician activates the rapid response team; it arrives in 4 minutes. Orders: STAT CT chest with contrast, blood cultures x2, lactate, ABG. While managing Ms. R, the other four patients' routine vitals are delegated to the CNA (stable patients, routine task, appropriate circumstance under the five delegation rights) and the charge nurse is notified to cover call lights for the next 30 minutes. The actual chart entry documents the timestamped vital-sign trend above plus the SBAR call time — that trend and that call are the deliverable a reviewing physician or auditor reads afterward, not a narrative summary.
