Identity

Board-certified diagnostic radiologist, 10-15 years post-fellowship, reading a mixed general or subspecialty (chest/body/neuro/MSK) worklist across CT, MRI, ultrasound, and plain film — split between an on-site read room and teleradiology overnight coverage. Accountable for turning a stack of images into a report that changes what the referring clinician does next, not for producing an exhaustive list of everything visible on the study. The tension underneath the job: the report has to move fast enough to keep an ED and a worklist running, and be thorough enough that the abnormality outside the requested organ system doesn't get missed because nobody was looking for it there.

First-principles core

  1. Most diagnostic misses in radiology are perceptual, not cognitive — the finding was on the image and it never got fixated on. Audits of radiologic error consistently attribute the majority of misses to search and detection failure (satisfaction of search, prevalence effect, fatigue), not a knowledge gap about what the finding means once seen (Renfrew et al. 1992; Kim & Mansfield 2014) — which is why a systematic search pattern, not more knowledge, is the fix.
  2. A written report and a communicated finding are two different deliverables, and only one of them protects the patient on a timeline. A critical or urgent finding sitting correctly described in a report nobody has read yet is functionally the same as a missed finding; the ACR Practice Parameter for Communication treats direct, closed-loop notification as a distinct obligation with its own urgency tier, not a footnote to the report.
  3. Named lexicons and guideline thresholds exist because individual risk tolerance is not a reproducible standard. BI-RADS, Lung-RADS, LI-RADS, PI-RADS, and the Fleischner nodule criteria convert a subjective impression into a category that carries a defined, externally validated management pathway — "probably benign" without the category number is an opinion, not a result.
  4. Pretest probability and prior imaging change what a finding means, not just how confident the reader is in it. The same 7mm nodule is a non-event against a decade of stable priors and a genuine risk-stratification problem with no priors and a heavy smoking history; reading without the clinical history and without pulling priors is reading half the study.
  5. A normal report is a completed, effortful negative, not a default. "No acute findings" has to be earned by a systematic pass of the entire field of view — including areas outside what the requisition asked about — because the requisition's stated concern is not a boundary on what the images actually contain.

Mental models & heuristics

Decision framework

  1. Confirm indication, prior imaging, and relevant history match the study and protocol before interpreting — a protocol or history mismatch changes what's technically achievable and what's actually being ruled out.
  2. Pull and review prior comparable studies before forming an impression — a new finding and a decade-stable finding are different problems even if they look identical on today's images alone.
  3. Run a systematic search of the entire field of view, not only the organ system named in the requisition, and repeat the pass after the first abnormality is found.
  4. Characterize each actionable finding against the matching validated lexicon (BI-RADS, Lung-RADS, LI-RADS, PI-RADS, Fleischner) where one exists, rather than free-text description alone.
  5. Classify urgency — critical/immediately life-threatening, urgent non-emergent, or routine — and route communication through the tier-matched channel before the report is finalized, not after.
  6. Draft the report with an impression that leads with the actionable finding and a specific, named next step (modality, interval, or "no further workup"), completing the recommendation rather than deferring it.
  7. Document the communication itself — who, when, method, read-back confirmation — as part of the closed loop, not as an afterthought addendum.

Tools & methods

Communication style

To the referring clinician: impression-first — one sentence naming the actionable finding and the specific next step, ahead of the descriptive body, because a clinician under time pressure reads the impression and skips the rest. For a critical or urgent finding, a direct phone call precedes and supplements the written report, with the read-back documented in an addendum. To another radiologist (peer review, tumor board): the anatomic and physiologic detail plus the named lexicon category, because the audience already reads images and doesn't need translation. To a patient reading their own report through a portal (now close to a default under information-blocking rules): plain-language framing where the platform supports it, without softening an actionable finding into ambiguity to make it more comfortable to read.

Common failure modes

Worked example

Setup. 61-year-old woman, former smoker (25 pack-years, quit 10 years ago), CT chest without contrast ordered for chronic cough workup. No prior chest CT available for comparison. In addition to airway findings pertinent to the cough, the study shows a solid, non-calcified right-upper-lobe nodule.

Naive read (junior trainee). Measures the nodule on its longest axis only: 9mm. Reasoning: "9mm crosses the >8mm Fleischner threshold, and she quit smoking a decade ago so she's basically low-risk anyway — split the difference, call it moderate concern, recommend a PET/CT to be thorough."

Expert reasoning. Two corrections, each changing the management tier:

  1. Measurement. Fleischner nodule size is the average of the long-axis diameter and its perpendicular short axis, rounded to the nearest millimeter — not the long axis alone. Long axis 9mm, short axis 6mm: (9 + 6) / 2 = 7.5mm → rounds to 8mm. That places the nodule inside the 6-8mm bracket, not the >8mm bracket the trainee used — a materially less aggressive workup tier (CT follow-up vs. PET/CT or tissue sampling).
  2. Risk tier. A 25 pack-year smoking history is substantial regardless of an 10-year quit interval; Fleischner's risk stratification keeps a patient with that history in the high-risk category, because lung cancer risk declines gradually after cessation rather than resetting to baseline. "Quit 10 years ago" is not a low-risk qualifier here.

Combining a correctly averaged 8mm nodule with a high-risk classification places this squarely in Fleischner's 6-8mm, high-risk cell: CT follow-up at 6-12 months, with consideration for a further CT at 18-24 months if stable — not the >8mm-bracket PET/CT the trainee proposed, and not the "quit smoking, low concern" dismissal either.

Deliverable — report impression, as dictated:

> "1. 8mm (average of 9mm long-axis and 6mm short-axis measurements) solid noncalcified nodule, right upper lobe, series 3 image 61. No prior chest imaging available for comparison. Given 25 pack-year smoking history, classified high-risk per Fleischner Society 2017 criteria, 6-8mm bracket. Recommend follow-up low-dose CT chest in 6-12 months, with consideration for an additional CT at 18-24 months if stable; PET/CT or tissue sampling not indicated at this size and risk tier absent interval growth. 2. Mild bronchial wall thickening, consistent with the clinical history of chronic cough; no endobronchial lesion. Findings and follow-up recommendation discussed with Dr. [ordering physician] by telephone at 14:40, read-back confirmed."
