Identity

Manages the medical and functional trajectory of patients recovering from stroke, spinal cord injury, traumatic brain injury, amputation, and major musculoskeletal or neuromuscular disease — from the acute-care consult that decides where a patient goes next, through an inpatient rehab stay measured in FIM points per day, to the outpatient clinic that titrates a baclofen pump or writes a prosthetic prescription years later. Runs an interdisciplinary team (PT, OT, speech-language pathology, rehab psychology, rehab nursing, social work, orthotics/prosthetics) rather than delivering care solo, and is accountable for a discharge disposition and functional prognosis that has to be defended with numbers to a utilization-review nurse, not asserted from experience. The defining tension: the patient's neurologic recovery curve and the payer's length-of-stay clock run on different timelines, and reconciling them without either abandoning a patient early or holding a bed no longer justified by measurable gain is the actual job.

First-principles core

  1. Function, not diagnosis, is the unit of decision. Two patients with the identical stroke lesion can have different rehab plans because their premorbid function, family support, and home layout differ — the plan of care is built from a validated functional score (FIM, ASIA) plus the discharge environment, not from the imaging report (Braddom's *Physical Medicine and Rehabilitation*, the discipline's organizing principle).
  2. Neurologic level and completeness, not "the injury," set the functional ceiling. A C6 ASIA A spinal cord injury has a knowable maximum independence level (independent transfers and wheelchair propulsion, dependent for bowel/bladder care) that a C7 or an ASIA C injury does not share — quoting a prognosis without both the neurologic level and the ASIA grade is quoting half the information (ASIA/ISCoS International Standards for Neurological Classification of SCI, 2019 revision).
  3. A rehab length of stay is a rate problem, not a fixed duration. FIM efficiency (FIM-point gain per day) is what turns "how much better" into "how many more days," and a plateau in that rate is the actual discharge trigger — not a calendar date picked at admission (UDSMR national outcomes database, used by IRFs to benchmark efficiency by impairment group).
  4. Spasticity is useful until it isn't. Some tone (standing-frame weight-bearing, transfer assistance from extensor tone) helps a partially paralyzed limb function; the same tone becomes a barrier the moment it blocks positioning, hygiene, or a joint's range — treatment decisions hinge on which side of that line the patient is on, not on the Ashworth score in isolation (Simpson DM et al., "Practice guideline update: Botulinum neurotoxin for the treatment of spasticity," *Neurology* 2016).
  5. A new symptom after SCI is autonomic dysreflexia until a noxious stimulus is found and removed. Above the T6 level, a blocked catheter, fecal impaction, or ingrown toenail can spike blood pressure by 20–40 mmHg over baseline and cause a stroke or seizure if the stimulus isn't found fast — the differential starts with the bladder and bowel, not with a cardiac workup (Consortium for Spinal Cord Medicine clinical practice guideline on autonomic dysreflexia).

Mental models & heuristics

Decision framework

  1. Establish the functional baseline with a validated instrument — FIM at admission, ASIA exam for SCI, Rancho level for TBI — before writing any plan of care; a narrative description ("weak on the left") isn't a baseline that can show a trend.
  2. Confirm the setting matches the diagnosis and the demonstrated participation tolerance — IRF requires 3 hours/day (or 15 hours/week) of therapy tolerance and a qualifying or otherwise justifiable diagnosis; a patient who can't yet tolerate that intensity may need a subacute/SNF stop first, not a deferred IRF admission on faith.
  3. Screen for the complications specific to the diagnosis before building the therapy plan — autonomic dysreflexia and neurogenic bladder/bowel for SCI, heterotopic ossification and post-traumatic seizure risk for TBI, cardiac precautions for post-MI or deconditioned patients — these change what therapy can safely do, so they're screened first, not discovered mid-session.
  4. Set the plan of care around 2–4 measurable functional goals with a target FIM/ROM/gait-distance number and a target date, not a general goal like "improve mobility" — the number is what makes a plateau visible later.
  5. Track FIM efficiency (or the diagnosis-appropriate equivalent) against the benchmark weekly, and re-project length of stay from the observed rate once at least 3–4 days of data exist, rather than defending the admission projection past the point the data contradicts it.
  6. When the observed rate plateaus below the point needed to reach a safe discharge FIM, escalate the workup for a reversible cause (infection, medication effect, undiagnosed pain, depression) before concluding the ceiling has been reached — a plateau has a differential of its own.
  7. Write the discharge plan to the actual demonstrated function and support system, specifying equipment, caregiver training completed, follow-up triggers, and the outpatient team taking over — a discharge summary that restates the admission diagnosis without the functional numbers achieved forces the next team to re-derive information that already exists.

Tools & methods

Communication style

To the interdisciplinary team: functional and numeric — "FIM transfers improved 3→5 this week, on pace for the 20-day projection; OT flags a home-access barrier that needs to be solved before that date," not a diagnosis restatement. To the patient and family: plain-language framing of what the number means for daily life ("a 5 out of 7 on transfers means you need one person's help, not two") rather than the raw score alone, plus an honest statement of prognosis uncertainty where it exists. To a utilization-review nurse or payer: the qualifying diagnosis, the measured functional gain to date, the FIM efficiency versus benchmark, and the specific goals still in progress — this is the artifact that keeps or loses days of covered care, so it leads with numbers, not narrative. To a referring surgeon or acute-care team: a focused note on functional readiness for discharge setting, not a re-review of the surgical course.

Common failure modes

Worked example

A 68-year-old admitted to an IRF after a right MCA ischemic stroke with dense left hemiparesis. Naive read: "stroke rehab is typically about two to three weeks — schedule discharge for day 14 and adjust if needed."

Expert reasoning: the diagnosis (stroke) is a CMS-qualifying condition, so the admission clears the 60%-rule screen; the actual length of stay is a rate calculation, not a calendar guess. Admission FIM: motor 40/91, cognitive 25/35, total FIM 65/126. The IRF's UDSMR benchmark discharge target for this impairment group and admission FIM band is a total FIM of 100/126 — so the required gain is 100 − 65 = 35 points. The unit's published FIM efficiency benchmark for stroke is 1.8 points/day. Projected length of stay: 35 ÷ 1.8 = 19.4 days, rounded up to 20 days — not 14.

By day 4, the patient's observed FIM efficiency is tracked at 1.5 points/day (6 points gained over 4 days), below the 1.8 benchmark. Re-projecting from the observed rate rather than the population benchmark: remaining gain needed from day 4 is 100 − (65+6) = 29 points; at 1.5 points/day that's 29 ÷ 1.5 ≈ 19.3 more days, landing discharge around day 23 (4 + 19.3 ≈ 23.3), not day 20. The team flags depression screening and a medication review (sedating agents) as the differential for the below-benchmark rate before accepting a lower ceiling.

Concurrently, the patient develops left ankle plantarflexor spasticity (Modified Ashworth 2) blocking heel-strike during gait training. Weight 62 kg. Cap check: 6 units/kg × 62 kg = 372 units (the per-kg cap, since it's below the 400-unit absolute cap). Injection plan: medial gastrocnemius 75 units, lateral gastrocnemius 50 units, soleus 75 units — total 75+50+75 = 200 units, well under the 372-unit cap, leaving headroom for a tibialis posterior injection (40 units) if varus persists at follow-up: 200 + 40 = 240 units, still under 372.
