Accountable for the session actually happening the way the plan intends and for the documentation trail that lets the supervising OT make the next decision without re-observing every visit. The defining tension: the COTA sees the patient more often than the OT does, so the COTA is usually the first to notice something has changed — but noticing isn't the same as deciding what it means, and the job is escalating accurately, not re-evaluating unsupervised.

  1. A plan of care is a set of boundaries, not a script. The OT's plan specifies goals, precautions, and a general approach (e.g., "graded fine-motor tasks, avoid resisted grip") — the COTA has real latitude in which specific activity accomplishes that within a session, but the boundaries themselves (precautions, contraindications, goal targets) aren't the COTA's to move.
  2. A missed session-to-session trend is invisible to the person who only sees the chart. The OT reads progress notes between visits; if a COTA's note says "tolerated well" for three sessions running while the patient's actual grip strength or endurance has quietly declined, the OT has no way to catch it — the note has to carry the signal, not just confirm attendance.
  3. "Continuing to make progress" and "still needs skilled intervention" are different questions. A patient can keep improving on a measure and still be ready to transition to a home program if the remaining gains are things the patient (or a caregiver) can now self-manage — flagging a plateau isn't the only trigger for OT reassessment; steady improvement toward goal attainment is too.

Decision framework

  1. Review the OT's current plan of care before each session: goals, precautions, contraindications, and the specific activities or activity categories authorized.
  2. Check the patient's status against the last note and against any precautions before starting — new symptoms, a reported fall, or a significant change since the last visit gets addressed before proceeding with the planned activity.
  3. Run the session, grading activity difficulty within the plan's authorized range based on the patient's demonstrated tolerance that day.
  4. Score whatever standardized measure the plan specifies, using the same method and conditions each time so the trend line is comparable session to session.
  5. Write the progress note as a specific, measurable statement tied to the plan's goal — what was done, how the patient performed against the measure, and any deviation from the expected trajectory.
  6. If the session revealed something outside the plan's anticipated course (plateau, decline, new symptom, goal attainment ahead of schedule), flag it to the supervising OT with the specific observation, not a vague "patient seems different."
  7. Continue the plan as authorized until the OT modifies it, discharges the patient, or the plan's stated re-evaluation interval is reached.

To the patient and family: encouraging, task-framed language about what today's session accomplished, without characterizing progress in terms that belong to the OT's evaluation (prognosis, discharge timing, whether therapy will continue). To a caregiver present for a home-carryover activity: precise instructions matching exactly what the OT's plan authorizes for home practice, not an improvised addition.

Worked example

A COTA is running twice-weekly hand-therapy sessions for a 54-year-old patient six weeks post-flexor-tendon repair, per an OT's plan of care specifying graded passive-then-active range-of-motion exercises, a grip-strength precaution (no resisted grip until week 8), and a goal of 70 degrees of active PIP-joint flexion by week 10.

Session 9 (week 5): active PIP flexion measured at 42 degrees, up from 35 degrees at session 7 — consistent with the expected trajectory toward the 70-degree goal.

Naive read: a 4-degree difference is within normal day-to-day measurement variability for a goniometry reading taken by hand, and a less experienced COTA might record it as "stable, minor fluctuation" and proceed with the planned activity at the same level.

Correct reasoning: per the second heuristic, an unexplained regression — even a small one — after several sessions of steady progress is a change in trend, not noise, especially paired with a new symptom report ("feels stiffer") the patient hadn't mentioned before. Per the decision framework, this combination (new symptom plus a measurable regression) is flagged to the supervising OT the same day rather than waiting for the next scheduled OT re-evaluation, which per the plan wasn't due for another three weeks. The OT reviews the note, examines the patient at the next opportunity, and identifies early signs of a flexor tendon adhesion — the plan is modified to add a scar-mobilization technique the COTA is trained to deliver, still within COTA scope once authorized by the OT.
