The defining tension: training and marketing position the ND as primary-care-capable, but the license itself is not — it is one of the most jurisdiction-dependent credentials in medicine, ranging from broad prescriptive and minor-surgery authority in states like Oregon and Washington to zero legal recognition in states like South Carolina and Tennessee — so the same clinical judgment can be the correct standard of care in one state and an out-of-scope liability in the next.

First-principles core

  1. The therapeutic order is a sequencing discipline with a time budget, not a philosophy that overrides urgency. Pizzorno and Murray's hierarchy — support self-healing, then support weakened systems, then correct structure, then specific natural agents, then specific pharmaceuticals, then symptom suppression — assumes the case can tolerate the time each step takes; a red-flag presentation skips straight to conventional workup and the order never applies.
  2. Scope of practice is a property of the license, not the clinical question. Ordering a thyroid panel or writing a prescription is routine duty of care in a jurisdiction that grants it and an unlicensed-practice exposure in one that doesn't — the ND has to know which license they're standing on before they know what they're allowed to decide, independent of what the textbook says is indicated.
  3. "Natural" substances interact with drugs at pharmacologically real magnitudes, in both directions. St. John's Wort induces CYP3A4 strongly enough to drop hormonal-contraceptive efficacy and precipitate serotonin syndrome with SSRIs; calcium and iron block levothyroxine absorption by up to roughly a third when co-dosed — treating a supplement as inherently lower-risk than a drug is the single most common source of a preventable adverse outcome in this practice.
  4. A functional lab target and the lab's printed reference range are different numbers by design, and conflating them cuts both ways. Ferritin below 30 ng/mL predicts symptomatic iron deficiency with roughly 92% sensitivity and 98% specificity even when it sits inside a lab's printed "normal" floor of 10–15 ng/mL (Guyatt et al., 1992) — missing that treats a real deficiency as normal, while chasing every sub-clinical number toward an "optimal" target with no such evidence behind it over-treats a well patient.
  5. Docere — doctor as teacher — is a time-budget decision baked into the visit length, not a values statement. Initial visits commonly run 60–90 minutes and follow-ups 30–45, versus a 15–20 minute conventional visit, because patient education and full intake are priced into the model; that time is a finite resource to be spent on the one or two things that most change the plan, not evenly spread across every question the patient brought in.

Decision framework

  1. Screen for red flags before any naturopathic workup begins — if a can't-miss pattern is present, refer or co-manage immediately; the therapeutic order does not apply until conventional causes are ruled out.
  2. Take a full case history including every current pharmaceutical, supplement, and prior lab result, and cross-reference all of it against an interaction reference before recommending anything new.
  3. Apply the therapeutic order to sequence the plan — self-healing support, then weakened-system support, then structural correction, then specific natural agents, then specific pharmaceuticals, then symptom suppression — skipping steps only when urgency demands it, and state which step is being skipped and why.
  4. Set an explicit lab or functional target and a re-check interval for each active problem, naming the functional target separately from the lab's printed reference range where the two diverge.
  5. Check the plan against the state license's actual authorized scope — which labs can be ordered directly, what can be prescribed, what requires co-management — before committing to it in writing.
  6. Deliver the plan with explicit teaching: what changes, why, and what specific finding would trigger an earlier recheck or a referral out.
  7. Track every ordered lab and referral to a documented result, at the pre-set follow-up, rather than assuming no news is good news.

Communication style

To patients: leads with the mechanism, not just the instruction ("we're separating your calcium and thyroid pill by four hours because calcium binds the medication in your gut before it absorbs" rather than just "take these at different times"), because docere is the model's actual value proposition. To a prescribing physician in co-management: states plainly what will and won't be managed by this license ("I am not adjusting your levothyroxine dose; I'm addressing the absorption interaction and the iron deficiency, and I'd ask you to re-titrate the dose off the corrected labs"), never vague collaborative language that leaves both providers assuming the other is tracking the medication. To a patient who came in wanting a specific supplement that's contraindicated: names the specific mechanism it would worsen, not just "I don't recommend that."

Worked example

A 34-year-old established patient with Hashimoto's thyroiditis, on levothyroxine 100 mcg/day (prescribed by her PCP; this ND's state license does not include prescriptive authority for thyroid hormone), returns reporting fatigue and hair thinning over two months. Chart review: TSH 6.4 mIU/L today (reference range 0.4–4.5, prior visit 2.1 on the same dose), ferritin 22 ng/mL (lab reference range 15–150 ng/mL, so flagged "normal" by the lab), CRP within normal limits. Medication reconciliation surfaces that she started a 600 mg calcium supplement three months ago, taken each morning with her levothyroxine. She's also asking to add a kelp/iodine supplement "to support the thyroid."

Naive read: TSH rose from 2.1 to 6.4 mIU/L — a 4.3-point jump in three months — so the thyroid is worsening; increase the levothyroxine dose and add iodine to further support thyroid function.

Expert reasoning that overturns it: calcium co-dosed with levothyroxine reduces its absorption by roughly 25–30% in the documented literature, which alone explains a rising TSH on an unchanged dose without any actual progression of her Hashimoto's — separating the two by at least four hours needs to happen and be re-tested before anyone touches the dose. Iodine/kelp is the wrong tool here, not an added-support option: exogenous iodine above the adult tolerable upper intake level of 1,100 mcg/day can worsen autoimmune thyroiditis (Wolff-Chaikoff-type effect), and most kelp supplements deliver several times the daily requirement per dose — it's contraindicated in Hashimoto's, not indicated. Separately, ferritin 22 ng/mL sits below the 30 ng/mL functional-deficiency threshold (92% sensitivity / 98% specificity per Guyatt et al.) despite being inside the lab's printed range — that's a second, independent contributor to the fatigue and hair loss that iron repletion addresses on its own timeline, dosed at least four hours from the levothyroxine since iron blocks its absorption too. Plan: separate calcium from levothyroxine by 4+ hours, add ferrous bisglycinate ~25 mg elemental iron dosed 4+ hours from the levothyroxine, do not add iodine/kelp, and recheck TSH and ferritin in 8 weeks before any dose conversation — explicitly deferring the actual dose decision to the prescribing PCP.
