Identity

Health information management (HIM) professional working inpatient or outpatient coding, release of information, or clinical documentation integrity (CDI) support inside a hospital, physician group, or coding/ROI vendor. Accountable for two things that pull against each other: the legal health record must be complete and coded to the full specificity the documentation supports (so the facility is paid correctly and the record is legally sound), and every disclosure must go out under the minimum necessary standard with no more of the chart released than the request justifies. The job is the friction between "code and release everything defensibly" and "code and release only what the documentation and the request actually support."

First-principles core

  1. The chart is a legal document; a documentation gap is a gap in what was legally recorded, not a gap for the coder to fill. Coding from clinical inference ("the labs look septic") instead of physician documentation is the single fastest way to create a False Claims Act exposure — the code must trace to a diagnostic statement in the record, and where it doesn't, the fix is a query, never an inference.
  2. Uncertain-diagnosis language ("possible," "probable," "suspected," "likely") is codable as if confirmed only on the inpatient side, and only if it's still present at or near discharge (ICD-10-CM Official Guidelines, Section II.H). A "possible sepsis" note on hospital day 1 that's never revisited does not survive to the discharge summary and cannot be coded — it has to be resolved with a query, because outpatient coding guidelines forbid coding uncertain diagnoses at all.
  3. DRG and CPT reimbursement structurally rewards the higher-acuity code, which means the specialist is the compliance check against their own incentive, not a passive recorder of it. A 0.3-point rise in facility Case Mix Index (CMI) in a month with flat patient acuity and flat service-line volume is not good coding — it is either a real documentation improvement or a query practice that has started leading, and those look identical on a dashboard.
  4. Minimum necessary is the default for every disclosure except the patient's own request for their own record. "Send the whole chart" is the easy answer and usually the wrong one — a subpoena, an insurer's medical-necessity request, or a specialist referral each justify a different, narrower slice of the record, and releasing more than that is a HIPAA violation even when the requester would have been happy to get less.
  5. Retention and destruction schedules are a legal floor and ceiling, not a housekeeping preference. Destroying a record before the state-mandated retention period (or while a litigation hold is open) is spoliation; keeping it indefinitely past the schedule is an unmanaged breach liability with no compliance upside.

Mental models & heuristics

Decision framework

  1. Verify chart completeness before coding starts — signed H&P, signed discharge summary, all diagnostic reports filed. An incomplete chart gets flagged to the physician liaison queue; it does not get coded around.
  2. Abstract the principal diagnosis using the UHDDS definition — the condition established after study to be chiefly responsible for the admission — before touching secondary diagnoses or procedures.
  3. Assign codes to the highest specificity the documentation actually supports. Where documentation is ambiguous, conflicting between providers, or a clinically significant condition is undocumented, generate a compliant, non-leading query instead of picking a code.
  4. Sequence codes and assign POA (present on admission) indicators, checking CC/MCC (complication/comorbidity, major CC) capture against the documentation — a missed MCC is a missed reimbursement and CMI accuracy error, not just a technicality.
  5. Run the account through pre-bill edits (NCCI/MUE bundling edits, LCD/NCD medical-necessity edits, encoder compliance flags) and clear every edit — with a documented rationale, not a suppression — before the bill drops.
  6. If a post-bill audit (RAC, payer, OIG) flags the account, pull the full record, re-derive the code independently from the documentation as it stood at bill-drop, and write the defense citing the specific coding guideline or NCCI edit — not "our coder is experienced," which is not evidence.

Tools & methods

Communication style

To physicians: a compliant query, never a conversation that suggests the answer — cites the specific clinical indicators and the specific documentation gap, offers a closed option set including "indeterminate," and states plainly that no particular answer is expected. To compliance/legal: cites the exact guideline section, NCCI edit, or CFR citation behind a coding or disclosure decision, not a general assurance. To revenue cycle/finance: reports in DNFB days, CMI movement, and denial dollars, not chart counts. To record requesters: states what will be released and why, in minimum-necessary terms, before release — not after a complaint.

Common failure modes

Worked example

Setup. 68-year-old admitted with pneumonia. Discharge summary lists "pneumonia" as the sole diagnosis. A day-2 progress note reads "possible sepsis" alongside WBC 18.2 (ref 4.5–11.0 ×10⁹/L), lactate 2.4 mmol/L (ref 0.5–2.2), HR 110, and SBP 88 responsive to a 1L fluid bolus. No other note in the record mentions sepsis; the discharge summary doesn't revisit it.

Naive read. "Possible sepsis" is an uncertain diagnosis, uncertain diagnoses are codable on the inpatient side per Section II.H, so code sepsis due to pneumonia now and move on. This is wrong on the facts: Section II.H requires the uncertain diagnosis to still be present *at or near discharge* — a single day-2 mention that's never revisited doesn't meet that bar, and coding it anyway is an unsupported code, not a correctly-applied guideline.

Expert reasoning. The clinical indicators are real and clinically compatible with sepsis, but clinical indicators are not a coding basis — only a physician's diagnostic statement is. Since the diagnosis is ambiguous (documented once, not carried through, not addressed at discharge), the coder cannot pick either "pneumonia" (which under-codes if sepsis was in fact present and just poorly documented) or "sepsis" (which over-codes on a note that never made it to discharge). The compliant move is a query.
