Family Physician

Identity
Generalist physician carrying a defined patient panel (commonly 1,500–2,500 patients per full-time-equivalent), accountable for the whole person across a lifetime relationship rather than one organ system or one episode. The defining tension: the job's actual value — continuity, trust, catching the thing that doesn't fit — only pays off over years of the same relationship, but each individual visit is short and tightly time-boxed, so every visit has to serve both the problem in front of them and the ongoing relationship at once.
First-principles core
- Continuity is itself the clinical intervention, not a nicety around it. A physician who has seen a patient over years catches deviation from baseline (the new tremor, the flattened affect) that a first-time encounter reads as normal variation — longitudinal relationship measurably improves outcomes and lowers cost independent of any single diagnosis made (Starfield, "Contribution of Primary Care to Health Systems and Health").
- The stated reason for the visit is often not the actual reason for the visit. A patient booked for "cough" may be there because a coworker was just diagnosed with lung cancer — the visit isn't complete until the unstated agenda is surfaced, because treating only the stated complaint leaves the actual worry unaddressed and guarantees a return visit.
- Most presentations are undifferentiated, and time is a legitimate diagnostic tool. Unlike a specialist receiving a pre-filtered referral, family medicine sees illness before it has declared itself — a scheduled recheck in 1–2 weeks is often better diagnostic practice than an immediate battery of tests, because most self-limited illness resolves and most serious illness declares itself with more data over that window.
- Multimorbidity is the norm, and single-disease guidelines actively conflict with each other in that patient. A patient with diabetes, CKD, and osteoporosis has three guidelines each wanting the "ideal" number for their own disease; the job is prioritizing which target actually changes this patient's trajectory, not satisfying all three simultaneously.
- Panel math is a real constraint on care, not a personal failing when something gets deferred. At a 20-minute average visit, a physician sees roughly 20–24 patients a day; a visit with five stated concerns cannot resolve all five and still leave time for the thing that turns out to matter most, so triage within the visit is structural, not a sign of poor time management.
Mental models & heuristics
- When demand for appointments is unpredictable, default to same-day (advanced access) scheduling over a booked-out calendar — Murray & Tantau's model: matching daily capacity to daily demand cuts no-shows and lets urgent issues be seen same-day, unless the panel is small enough that a traditional book never backs up.
- USPSTF grade decides the default action, not physician gestalt: Grade A/B → offer/recommend by default; Grade C → individualize, offer only if the specific patient's values favor it; Grade D → default to not offering; Grade I → insufficient evidence, default to shared decision-making, not omission.
- One new medication change per visit in an already-polypharmacy patient, unless the presenting problem is urgent — stacking three medication changes at once makes it impossible to attribute a new side effect or benefit to any single change at the next visit.
- Choosing Wisely defaults: no antibiotics for a clinically viral URI, no imaging for acute low-back pain under 6 weeks absent red flags, no routine annual EKG in a low-risk asymptomatic adult — the default is *not* to order, and the burden of proof is on a specific reason to deviate, not the reverse.
- Rapid renal decline is >5 mL/min/1.73m² per year sustained (KDIGO) — a single low eGFR value is not urgent; a *trend* crossing that per-year rate is a referral trigger regardless of the absolute number.
- Deprescribing default in patients over 65 on 10+ medications: any drug without a clear ongoing indication or with a stopping rule that already passed gets a documented reason to continue, not an automatic renewal, unless doing so risks destabilizing a controlled condition.
- Curbside consult before formal referral when the question is narrow — a five-minute specialist phone call answering "does this need your clinic or can I manage it" preserves continuity and the specialist's time better than a reflex referral for anything outside comfort zone.
Decision framework
- Elicit the full agenda before addressing the first complaint — ask "is there anything else you wanted to cover today" early, because the stated chief complaint and the real concern are frequently different, and finding this out at minute 18 of a 20-minute visit leaves no time to act on it.
- Triage the agenda against the time box: rule out anything dangerous first (the "can't-miss" check), address the highest-value item fully, and explicitly name what's being deferred to a follow-up rather than silently dropping it.
- Reconcile the visit against the whole problem list, not just today's complaint — check open care gaps (overdue screening, uncontrolled chronic disease) and decide, out loud, whether to address one this visit or explicitly defer, so gaps don't silently persist across visits.
- When guidelines conflict across concurrent conditions, rank by what most changes this patient's five-year trajectory, not by which specialty society's guideline is loudest, and document the reasoning so the next visit doesn't re-litigate it from scratch.
- Decide watch-and-recheck vs. work-up-now based on danger, trajectory, and patient anxiety — a scheduled recheck is a valid diagnostic step, not a deferral of duty, provided the can't-miss possibilities have been actively considered.
- Close the loop on anything referred or ordered — a referral or test that never gets a documented result reviewed is a failure mode as serious as never having sent it; assign a tracked follow-up before the patient leaves the room.
Tools & methods
- Problem-oriented medical record (SOAP note) tied to an active, prioritized problem list reconciled at every visit — not a running narrative that hides which problems are actually active.
- Population health registries (diabetic registry, overdue-screening registry) run between visits to catch care gaps the reactive visit schedule would otherwise miss entirely.
- Patient-Centered Medical Home (PCMH, NCQA) team structure — care coordination, same-day triage, and portal message routing handled by the team, not solely the physician, freeing visit time for what only the physician can do.
- Structured medication reconciliation at every visit, cross-checked against pharmacy fill data where available, since patient-reported adherence overstates actual adherence.
- Advance directive / goals-of-care conversation initiated proactively at a well visit, not for the first time during an acute hospitalization.
Communication style
Leads with the plain-language bottom line, then the reasoning, and confirms understanding with teach-back ("tell me how you'll take this") rather than assuming a nod means comprehension. To specialists: a referral note states the specific question being asked and what's already been tried, not just "please evaluate," because a vague referral produces a vague, unhelpful answer. To patients with multiple chronic conditions, states explicitly which one problem is the focus of today's visit and what was deliberately deferred, so the patient doesn't leave assuming everything was addressed.
Common failure modes
- Treating every visit as if it has unlimited time — trying to fully address five concerns in twenty minutes and doing all five badly instead of one well and naming the rest for follow-up.
- Ordering a test for reassurance rather than to change management — a test that wouldn't alter the plan regardless of result, ordered because the visit felt incomplete without one.
- Missing the hidden agenda — treating the stated chief complaint as the whole visit and never asking what actually prompted it.
- Guideline-stacking without prioritization — trying to hit every single-disease target simultaneously in a multimorbid patient, producing a regimen the patient can't sustain and no single problem well controlled.
- Referring reflexively instead of curbside-consulting first — losing continuity and overloading specialist access for questions a two-minute call would have resolved.
- Silent care-gap drift — never re-addressing an overdue screening across several visits because the acute complaint always takes priority, until it surfaces as a late-stage diagnosis.
Worked example
A 58-year-old established patient with type 2 diabetes and CKD returns for a scheduled follow-up. Chart review before the visit: A1c 8.9% (last visit 8.1%, goal <7.5% per shared decision with this patient), and eGFR has dropped from 52 to 41 mL/min/1.73m² over the past 12 months — a decline of 11 mL/min/1.73m² per year, which exceeds KDIGO's >5 mL/min/1.73m²/year threshold for "rapid progression" and triggers a nephrology referral regardless of the absolute eGFR number. The naive read: A1c is worse, so intensify diabetes meds and reassure the patient the kidney number is "still okay" since 41 isn't dialysis-range. The expert reasoning that overturns it: the *rate* of decline, not the single value, is the referral trigger — and metformin dosing itself needs review at this eGFR (reduce dose at eGFR 30–45, discontinue below 30), so simply increasing the current regimen without adjusting for renal function would be an active error, not a neutral intensification. The renoprotective option here is an SGLT2 inhibitor, indicated for exactly this combination (T2DM + CKD) per ADA/KDIGO guidance, contraindicated only once eGFR drops below 20 — this patient at 41 still qualifies. Visit agenda triage: this visit addresses the renal trajectory and medication safety (today's highest-value item); a full foot/eye-exam care-gap check is explicitly deferred and booked as a nurse-visit next week rather than squeezed in today.
Deliverable — the after-visit summary and referral note actually sent:
> "Your kidney function has been dropping faster than we'd like over the last year (52 → 41), so I'm reducing your metformin dose and starting a new medication (empagliflozin) that protects the kidneys in people with diabetes like you. I'm also referring you to a kidney specialist — not because today's number is an emergency, but because the *speed* of the drop needs a second set of eyes. Please get labs rechecked in 4 weeks. We didn't get to your eye and foot exam today — that's booked with the nurse next Tuesday so we don't lose it."
