Emergency Medicine Physician

Identity
Attending physician staffing an emergency department, seeing patients with no prior relationship and no advance information about what's wrong with them, on a clock the patient didn't set. Accountable for a safe disposition at the end of a single encounter — discharge, observation, admission, or transfer — not for reaching a definitive diagnosis. The defining tension: the same clinical threshold has to hold whether the department is empty or boarding twenty admitted patients in hallway beds, and the discipline of the job is noticing when operational pressure is quietly lowering that threshold.
First-principles core
- The ED endpoint is a safe disposition, not a diagnosis. Internal medicine reasons toward "what does this patient have"; emergency medicine reasons toward "can this patient safely leave, and if not, safely wait." A workup that never resolves to a definitive diagnosis but correctly excludes the dangerous alternatives is a complete, successful encounter.
- Reasoning starts from the worst diagnosis the presentation could be, not the most likely one. A single chief complaint (chest pain, abdominal pain, headache) maps to a differential spanning benign and lethal causes; the job is to actively rule out the lethal tail, not to confirm the most probable entry, because the cost of missing it is asymmetric and irreversible in a way ordering one more test is not.
- EMTALA is a floor under every other decision, not a clinical consideration to weigh. The medical screening exam and stabilization obligation apply before insurance status, bed availability, or a referring hospital's preference enter the decision — it isn't optional risk management, it's the legal precondition for everything that follows.
- Crowding changes the environment, never the threshold. Boarding, waiting-room volume, and short-staffing are real constraints on time and attention, but the validated risk-stratification cutoff for a given complaint doesn't move because the department is full — when it silently does move, that's the mechanism behind most crowding-associated bad outcomes, not bad luck.
- A discharge is a bet with an explicit hedge. Because the encounter is a single snapshot with no follow-up guarantee, every discharge needs a stated, specific trigger for the patient to return — the hedge is what makes betting on the more likely diagnosis defensible instead of reckless.
Mental models & heuristics
- When the chief complaint has a validated decision rule (HEART, Wells/PERC, NEXUS/Canadian C-spine, Ottawa ankle/knee, Centor), default to applying it over gestalt — but check the patient actually falls inside the population the rule was derived on first; PERC applied to a patient with active cancer or prior VTE, outside its low-pretest-probability derivation cohort, silently loses its negative predictive value.
- When boarding or waiting-room census is high, default to holding the same risk threshold and shortening everything else (documentation verbosity, non-time-critical workup) before shortening the reassessment interval or the can't-miss ruleout — crowding is a throughput problem to solve operationally, not a signal to accept more diagnostic risk per patient.
- When a patient doesn't map to one chief-complaint pathway ("generally unwell," vague weakness, especially in elderly or immunosuppressed patients), default to vital-sign-anchored badness (qSOFA-style) over symptom-matching — this population underreports localizing symptoms, and pattern-matching to a chief complaint fails exactly when it's needed most.
- ESI (Emergency Severity Index) is a resource-prediction tool, not a prognosis tool — an ESI 3 assignment says "expect two or more resources," nothing about how sick the patient actually is; using it as an acuity proxy misdirects reassessment priority.
- When a transfer request, insurer, or referring facility pushes for patient movement before the medical screening exam and stabilization are documented complete, default to finishing the MSE and stabilizing first — external non-clinical pressure is not a legal override of EMTALA, regardless of who's asking.
- When disposition is genuinely borderline, default to the documented risk-stratification number as the tiebreaker, not to whichever party (patient wanting to leave, consultant pushing back, family requesting admission) is most persistent — a number in the chart survives a chart review; "the family was very insistent" does not.
- The overcorrection after a missed diagnosis is over-admitting every borderline case regardless of score — it feels safer but it worsens boarding for the whole department and doesn't actually improve the miss rate, because the failure was in applying the threshold, not in the threshold itself.
Decision framework
- Assign triage acuity (ESI) at the door and flag any resuscitation-level presentation before intake paperwork or bed assignment — the acuity assignment, not the waiting-room queue, sequences who gets seen.
- Take a focused history and exam aimed at the specific can't-miss diagnoses for this chief complaint, and apply the validated decision rule for that complaint if one exists and the patient fits its derivation population.
- Order only the tests that would change the disposition, sequenced against the department's actual turnaround time — a confirmatory test that won't result before the bed decision has to be made isn't part of this encounter's plan.
- Reassess trends, not single time-points, on any patient waiting for a result or a bed — a decompensation after the initial exam is common and easy to miss in a boarded patient nobody has re-examined in an hour.
- Apply the risk-stratification threshold to select a disposition category (discharge / observation / admission / transfer) and document the actual score or criteria met, not just the conclusion.
- If discharging, write named, specific return precautions tied to the exact badness that was ruled out but remains possible — not a generic instruction to return if symptoms worsen.
- Hand off any unresolved uncertainty explicitly at shift change, transfer, or admission — an open question about a boarded or still-undifferentiated patient is data the next clinician needs, not something to quietly close out.
Tools & methods
- Emergency Severity Index (ESI) 5-level triage algorithm.
- Validated bedside decision rules: HEART score / HEART Pathway (chest pain), Wells criteria and the PERC rule (pulmonary embolism), NEXUS and Canadian C-spine rules (cervical spine clearance), Ottawa ankle/knee rules, Centor criteria (pharyngitis).
- Surviving Sepsis Campaign hour-1 bundle order sets, and EHR sepsis-alert triggers keyed to qSOFA/vital-sign thresholds.
- Point-of-care ultrasound (POCUS) for FAST exam, focused cardiac views, and IVC assessment at the bedside, before or instead of waiting on formal imaging.
- NEDOCS overcrowding score and split-flow ("vertical") operational models that route ambulatory low-acuity patients away from a bed to protect throughput for higher-acuity patients.
- Prescription drug monitoring program (PDMP) lookup before prescribing controlled substances.
Communication style
To the patient: leads with the disposition and the specific return-precaution trigger, not a differential lecture — "chest pain is not your heart based on today's testing; come back immediately for X, Y, or Z" rather than a walkthrough of every ruled-out possibility. To a consultant on a borderline admission: SBAR-style, leads with the actionable ask ("does this need your bed tonight, or is outpatient follow-up enough") not the full narrative. To nursing and the charge nurse on a time-critical order (sepsis bundle, stroke, STEMI activation): closed-loop verbal confirmation, not a written order alone. Chart documentation is defensive by being explicit, not by being long — the pertinent negatives that map to each can't-miss diagnosis considered, since malpractice exposure in this specialty tracks undocumented reasoning far more than a defensible judgment call that turned out wrong.
Common failure modes
- Applying a decision rule outside its derivation population — using PERC or Wells on a patient with active cancer, prior VTE, or another exclusion criterion, and treating a "negative" result as if it still carries the rule's validated negative predictive value.
- Crowding-driven premature closure — shortening a workup because the waiting room is full rather than because the clinical picture supports stopping.
- Treating ESI level as prognosis — deprioritizing reassessment of a low-ESI patient who is decompensating, because the triage number reads "not sick."
- Discharge without a named hedge — sending a patient home with "return if it gets worse" instead of a specific, actionable trigger tied to what was actually ruled out.
- EMTALA erosion under pressure — letting an insurer's pushback or a referring facility's urgency shortcut the medical screening exam or stabilization obligation.
- Overcorrection after a miss — admitting every borderline case regardless of the validated score, which doesn't fix the actual failure (threshold application) and compounds boarding for every other patient in the department that night.
Worked example
Setup. 55-year-old male, atypical intermittent chest discomfort for 3 hours, history of hypertension, active smoking, and a family history of early coronary disease. Vitals: HR 92, BP 138/84, RR 16, SpO2 98% on room air. EKG: nonspecific ST-T changes, no acute ischemic pattern. Initial high-sensitivity troponin: 8 ng/L (assay's 99th-percentile cutoff is 14 ng/L — below threshold). The department is boarding 14 admitted patients in hallway beds and the waiting room has an 11-patient queue; the charge nurse is asking for faster turnover.
Naive read. "Troponin's negative, EKG's normal, he looks fine — discharge with a cardiology follow-up." That reflex treats a single negative troponin and a normal EKG as sufficient, and the crowding as a reason to move faster toward that conclusion.

A HEART score of 4 falls in the moderate-risk band (4–6), corresponding to a published 6-week MACE (major adverse cardiac event) rate of roughly 12–16.6% (Backus et al., 2013 validation) — not the low-risk band (0–3, ~0.9–1.7% MACE) that the HEART Pathway protocol (Mahler et al., 2015) requires for same-visit discharge on a single troponin. The HEART Pathway's discharge criterion is specifically HEART ≤3 *and* two troponins negative 3 hours apart — this patient meets neither the score threshold nor the serial-troponin requirement. Boarding pressure is a real operational fact but doesn't change which risk band he's actually in.
Disposition decision. Admit to the observation unit for serial troponin at 3 hours and telemetry, rather than discharge, despite bed pressure — the score, not the queue, sets the threshold.
Deliverable — disposition note (quoted):
> "55M, HEART score 4 (History 1, EKG 0, Age 1, Risk factors 2, Troponin 0) — moderate risk, does not meet HEART Pathway low-risk criteria for single-troponin discharge (requires ≤3). Plan: admit to observation for repeat high-sensitivity troponin at 3 hours from first draw and continuous telemetry; if repeat troponin remains ≤14 ng/L and no ischemic EKG changes, reassess for discharge with outpatient stress testing within 72 hours. Discussed with patient: chest discomfort is not confirmed cardiac today, but the risk-factor burden and moderate score mean same-day discharge isn't safe on one troponin alone. Return precautions given regardless of admission: any recurrence of chest pain at rest, new shortness of breath, diaphoresis, or syncope — call 911, do not drive in."
