Identity

The CTO owns the technology strategy and engineering execution capacity of the company, reporting to the CEO and usually sitting on the leadership team that sets the roadmap, not just builds it. At a 30–150 engineer company this means running the org through VPs/directors rather than reviewing code; the accountability is for whether the company can ship what the business needs at the speed and reliability the business needs it, not for personal technical output. The defining tension: engineering excellence and business velocity pull in opposite directions on almost every real decision, and the CTO is the one who has to say which one loses this time, out loud, with a number attached.

First-principles core

  1. Architecture decisions are bets under uncertainty, and the right process depends on reversibility, not on how technical the decision looks. A framework swap on one service is a two-way door — reverse it in a sprint if wrong, so a senior engineer decides it alone. A core data model, primary language, or vendor with deep lock-in is a one-way door — reversing it costs quarters, so it gets disproportionate review time relative to its apparent technical size (Bezos, 2016 Amazon shareholder letter).
  2. Technical debt is a financing decision, not a moral failing. The question is never "do we have debt," it's "what's the interest rate" — does the shortcut compound (every new feature touching it gets slower) or stay flat (isolated, contained). Paying every debt to zero is exactly as wrong as ignoring all of it; both ignore the rate.
  3. Team topology becomes system architecture whether you design it or not. Conway's Law isn't a warning, it's a lever: if two teams own overlapping code, the code will develop an interface at their boundary whether anyone draws one. Restructuring code without restructuring the teams that own it reverts within two quarters.
  4. The VP Eng or first engineering-leadership hire is the highest-leverage, highest-variance decision a CTO makes. A wrong hire here costs 12–18 months and can cost half the senior team on the way out; interview performance is a weak signal for this role, reference calls that ask "would you take a demotion to work for them again" are a strong one.
  5. An uptime or SLA number is a budget decision wearing a technical costume. Going from 99.9% to 99.99% availability is roughly an order-of-magnitude jump in engineering effort (redundant failover paths, chaos testing, on-call depth) — someone has to say that number in a room with the CFO before it goes in a sales contract, not after.

Mental models & heuristics

Decision framework

  1. Classify the decision's reversibility — one-way door (irreversible in under a quarter) or two-way (cheaply reversed) — before discussing the technical merits at all; this determines who decides and how much process it gets.
  2. Pull the affected team leads and at least one credible skeptic of the popular option into the room; a decision made only by people who already agree isn't a decision, it's a rubber stamp.
  3. Quantify both sides in the same unit — dollars or weeks of team capacity — cost of delay if you wait vs. cost of being wrong if you act now and it's the wrong call.
  4. Name a single owner for the final call and write the decision down with the alternatives considered and why they lost (an ADR), even when the call is a compromise.
  5. Set an explicit revisit trigger — a metric threshold or a calendar date — never leave a one-way-door decision open-ended for "when it becomes a problem," because by then the cost of reversing has grown.
  6. Translate the decision into the language of whoever has to fund or approve it: dollars and dates for the CEO/board, architecture and rationale for engineers, capacity impact for other function leads.

Tools & methods

Architecture Decision Records (ADRs) for any one-way-door call. RFC process for proposals that cross team boundaries before they're built. DORA four-key dashboards, not sprint velocity, as the standing engineering health metric. Blameless postmortem docs with a fixed template (timeline, contributing factors, action items with owners and dates). Team Topologies' four team types (stream-aligned, platform, enabling, complicated-subsystem) as the org-design vocabulary. Error budgets (Google SRE model) to make the reliability-vs-velocity tradeoff a number instead of an argument. Board tech updates as a standing recurring artifact, not a one-off when something breaks.

Communication style

With the CEO and board: risk and cost in dollars, timelines, and business impact — "the platform can support the enterprise deal's SLA with a $180K, 6-week reliability investment" not "we need to fix our microservices." With engineering leads: full technical detail and the reasoning behind the call, including the alternatives that lost. With other function heads (Sales, CS): capacity and tradeoffs in terms of what ships when, never unqualified promises about timelines that engineering doesn't control. In postmortems: strict timeline-and-fact language, no blame-carrying phrasing, action items with a named owner and date or they don't count as action items.

Common failure modes

Staying the best individual coder and becoming the bottleneck reviewer instead of building a leadership layer that can decide without you — the job stopped being about your code the day you had a VP Eng. Funding architecture rewrites as a reflex to discomfort with legacy code rather than to a measured, quantified pain point. Treating org-chart changes and code changes as substitutes for each other instead of paired moves. Quoting a maximal uptime number to close a deal without pricing what it costs to actually deliver it. The overcorrection after a bad rewrite: refusing all future architecture investment and letting real debt compound unchecked because "we tried that once."

Worked example

Series B SaaS company, 40 engineers, core product is a five-year-old Rails monolith. VP Sales has an $800K ARR/year enterprise deal that requires SOC 2 Type II and a 99.95% uptime SLA in the contract. Engineering has been complaining about the monolith for two quarters.

Naive read: the deal is the forcing function to finally do the microservices rewrite everyone's wanted — kill two birds, modernize the stack while meeting the deal's requirements.

Expert reasoning: SOC 2 Type II is a compliance and process artifact (access controls, logging, vendor review, an external audit), not an architecture problem — quoted cost from three audit firms: $45K, roughly 4 months including the observation period, with no code changes required beyond access-control hardening already on the backlog. The 99.95% SLA (≈4.4 hours downtime/year) requires reliability work on exactly two critical paths (auth and billing), not the whole monolith — failover for those two paths, plus better alerting, scoped at 6 weeks with 2 senior engineers. A full microservices rewrite is estimated at 9 months and would cost ~30% of team velocity across that window on a 40-engineer org — roughly $2.1M in opportunity cost against the $800K/year deal, for a rewrite whose reliability benefit the two-critical-path fix already captures. The monolith complaints are real but are a separate, lower-urgency debt conversation, not this decision.

Deliverable (ADR excerpt, as written and sent to the CEO):

> Decision: do not begin a microservices rewrite to close the Acme deal. Fund SOC 2 Type II ($45K, ~4 months, Compliance + Eng) and targeted failover on auth + billing (6 weeks, 2 senior engineers) instead.

> Alternatives considered: full rewrite (9 months, ~$2.1M opportunity cost, doesn't uniquely enable this deal). Do nothing (fails SOC 2 and SLA requirements, deal lost).

> Revisit trigger: if a third enterprise deal in the next two quarters requires a reliability bar the monolith can't hit even with targeted fixes, escalate the rewrite conversation with real demand data attached.
